Consent to personal data processing

I, as a visitor to polapam.ru, freely, of my own will and in my own interest, give my consent to Anton Filimonov (Tax ID 631816218908, address: 91 Lva Tolstogo St., Samara, hereinafter “the Operator”) to process my personal data on the terms set out below.

1. List of the personal data to which this consent is given

• surname and first name;
• email address;
• phone number;
• pet information (name, breed, age, sex, details of vaccinations and veterinary visits);
• information about the specialist's qualifications and work experience, portfolio of work;
• data about the region where services are provided;
• any other data I voluntarily provide when filling in forms on the website.

This consent covers processing both by automated means and without such means.

2. Purposes of processing

• processing my requests and providing feedback;
• connecting pet owners and specialists, posting and searching for service listings;
• registration and sign-in to the personal account;
• conclusion and performance of contracts for the provision of services;
• notifications about the status of requests and bookings;
• improving the quality of the service and of the user experience;
• compliance with the requirements of the legislation of the Russian Federation.

3. List of the actions with personal data to which consent is given

Consent is given for the following actions: collection, recording, systematisation, accumulation, storage, clarification (updating, modification), extraction, use, blocking, deletion and destruction — both by automated means and without such means.

4. Term of the consent and the procedure for withdrawing it

4.1. The consent is valid from the moment it is given until the purposes of processing are achieved or until it is withdrawn.

4.2. I may withdraw the consent at any time by sending a written statement to the address support@polapam.ru or through the website interface (profile settings → account deletion).

4.3. The statement must indicate: surname and first name, email address and phone number, date and signature. The statement may be sent in electronic form with identity verification.

4.4. After the consent is withdrawn, the Operator stops processing the personal data and destroys it within a period not exceeding 10 working days, unless different retention periods are established by the legislation of the Russian Federation.

5. Specifically about advertising messages

5.1. The “Consent to receiving advertising messages” parameter on the website is independent of this Consent.

5.2. Until the advertising consent has been given, the operator does not send advertising messages. The advertising consent may be withdrawn at any moment.

I confirm that:

• the data I have provided is accurate and belongs to me;
• I have given this consent voluntarily.
